We’ll require some information to complete your company’s setup for the Bitcoin Savings Plan. Before you get started, you'll need the following:
1. Business Information
- General business information and registration number
- Company registration documents, such as certificates of incorporation or articles of association
2. Directors and Ownership Structure
- Names and addresses of company Directors
- Names and addresses of Ultimate Beneficial Owners: Individuals who own 25% or more of the business
Please list the names of all the Directors, Trustees, Known Beneficiaries, Settlors and other Principals of the Entity, as the case may be for that particular type of Entity.
If you have more than 4 Directors, please attach a file below.
If you have more than four Directors, please attached a document with each Director's First Name, Last Name and Address.
A beneficial owner is an actual person who owns or controls, directly or indirectly, 25% or more of the Entity's shares of capital stock (this includes Sole Proprietors). If a beneficial owner is another Entity, please list the names and addresses of the beneficial Owners in that Entity, as well as the Entity’s address.
There are no actual persons who own or control, directly or indirectly, 25% or more of the Entity's shares of capital stock in the Entity. Continue to next page.
